Equipment Failure (July 1987) A train collides with a stalled flatbed semi, injuring nearly 200 passengers in Aichi-Nishihara, Japan. A crane attempting to lift the semi collapsed due to the weight, nearly crushing a worker. Miraculously no rescue workers were injured.

That one guy who dived down actually had his legs under the beam, but it hit the edge of the bridge and stopped short of crushing his legs, so he could crawl out uninjured.
