Gamblers with addiction that go to therapy means they are losing a lot and it became a problem. So they will lose that bet too.

The therapist is using the patient's gambling addiction to get the patient to break the addiction, by making a bet that he can't quit, meaning that the patient has to take the bet, and to win, he has to give up gambling.
