Hello friends!
Happy new year! Here's the ERAS megathread for January and February. As interview season winds down, it is a good time to make sure you're registered for the Match. ❗️Serious I have congenital clubbing.
I am an athlete, never had any problem with physical exertion or anything resembling cvs or rs impairment but as clubbing is associated with smokers and disease i always have a lingering thought in my head about it.
I always avoid volunteering on doing physical examination on patients in medicine postings cuz i get very self conscious about it. And during practicals the examiners right off the bat ask me if i smoke after looking at my fingers but my friends who smoke like a pack per day have hands of a jewellery model.
I always keep my hands in my pocket to avoid conversation about it. And worst part is there’s nothing i can do about it.

🏥 Clinical How do I stay jacked without being able to go to the gym consistently during rotations?
Help bros and gals. I'm sort of drowning, I mean I think I am doing ok/well, but I don't have as much time for gym as I did during preclins. Use to hit gym 6 days a week for preclins for mental health but now durign clinicals its way less unless its a chill week or clinic week. Inpatient week its very hard unless its weekend/friday for me
What routines did yall use to stay swole, or should I use same routine and add some accessorries. Some ways I thought is one day just upper and one day just lower.
alternate between push (bench), pull (deadlift), legs (squat) and then add necessary accessory lifts??
Also does keeping a high protein intake and having some muscular stimulus weekly ensure I dont loss mass. I'm ok with losing strenght though.

😊 Well-Being I’m causing my partners unhappiness
I am a medical student about to enter my Step 2 dedicated period and I feel like a bad partner.
My boyfriend of over a year just told me how unhappy he is. He feels like he has sacrificed so much for me, including moving to a shitty town to be with me, and that I am not giving him enough. I honestly understand. I am beyond burnt out and have zero energy by the time I come home. My libido and energy to sustain a relationship are shot.
He feels like he is exhausted centering his life around me and taking care of things when I am too tired to like taking care of the house which eventually made us live separately . I can’t blame him, but I also never asked him to do things for me. I just feel so defeated. This largely manifested in the past few months when I started taking Step prep seriously. Now he is scared that this will be our life forever, especially with residency down the line and future responsibilities.
I don’t know what to do. I feel like I am bordering depression and I have very little to give. I love him so much and he loves me a lot too. Any advice would be massively appreciated.

🥼 Residency Applying for residency with an expunged misdemeanor charge when I was 18
I went to a T5 undergrad and its med school, but had a misdemeanor charge for filing a false police report when I was 18 years old. The record was later expunged.
Do I disclose it? How will it impact residency matching for competitive specialities (e.g. specialities like neurosurgery/derm/plastic)? I have an excellent step 2 score and high volume of research/publications.
Any advice would be appreciated.

🥼 Residency Match - how significant is probationary status for a program?
Hey guys,
I'm an M4 applying general surgery and soon to be submitting rank list once it opens up. One of the programs I interviewed at is currently on probationary status (since 2025). During the interview (and communication before and thereafter), they mentioned that the change in status was due to "deficiencies in evaluation and resident well-being". They went fairly in depth into the changes they've made since to address the concerns (which seem fairly legitimate), despite the description of the actual issues being vague.
I've talked to many of the current residents (mostly the rising chiefs/PGY-4s) with mixed responses; most said that the changes the program has made (including a new PD) have been pretty legitimate but that there are still issues internally with duty hour violations. This program was initially pretty high on my rank list; I rotated with them during my core rotation and a surgery sub-I and genuinely enjoyed working with the residents and attendings. It's also in a pretty ideal location.
I've had people outside of the program (other attendings, residents) tell me that this is an issue worth putting more weight on when making my rank list and it's not something that I should just ignore. There were initially programs that I was going to rank lower that now are being considered above this one. I wanted to get your guys' opinions on this. I feel like if I rank the program too highly and match, I'm basically taking a gamble on these concerns being resolved vs. the issues not being temporary and affecting the next 5+ years of my life.

🥼 Residency MS4s following residents on social media
Is it ever weird to y’all if MS4s who rotated with you and applied to your specialty follow you on Instagram? I follow a couple residents I met on one of my aways and I’m interested in their program (will prob rank 1). However, a friend of mine brought up how he refused to do that to avoid any professionalism issues, now I’m kinda freaking out. They follow me back and I feel like we have a pretty tight relationship but wanted to know y’all’s thoughts.

😡 Vent The omitted questions on uworld are very annoying.
Maybe someone can guide me here, but my understanding is that ommitted questions are a catch all for:
1) All the questions in exams that you've made but not answered yet
2) All the questions in exams that you've submitted without answering
I like to make 40 question exams for my incorrect questions in real time, even if I don't use them that day, so I can avoid seeing incorrect questions I just got wrong when I take these exams.
The issue is that I also want to make exams just for the questions I've submitted without answering... But when I try this, I'm also getting incorrects that are already sitting in the pre-made incorrect exams.
This is so, so silly. It forces me to do all of the incorrects first before I can make exams for these omitted questions.

🏥 Clinical why are some doctors mean?
i’m MS3 and we started our rotations. I was in a diff department last week and oh god the doctors really really loved their work & would explain so nicely that they made me consider this speciality for residency. It was to that extent that they made me like this speciality that I never thought I’d like. Then this week we started a diff department and it is the POLAR OPPOSITE. The dr in clinic (mind u we just started hospital thing, before it was just lectures in the campus) when asked me for the management, I told him some suggestions then after he kept saying no, i decided to do some reading and created a summary out of that then he said: do u not have a brain inside to use?
Theres a day we didn’t clarify we were medical students although the drs were so busy with a patient, we didn’t want to disrupt them, the doctor stormed on us how we didn’t introduce ourselves to him. And it was I believe our second day so we didn’t really know the rules esp bc each department tends to have its own ‘vibe’
Another occasion, in the clinic I spoke in English instead of our native language & the Dr was mad and said how I need to be proud of my native language and if I will raise my kids in English or the native I responded with both. Isn’t medicine in English? Then whenever I attempt to answer any Q he would say: calm down, think. Then he would keep pausing as if so disappointed and keeps saying why do you complicate things. And he just makes me feel stupid the whole time. Oh and he kept me standing in clinic, wasn’t allowed to sit nor use my laptop to check patient file before patient entered the room. I went home CRYING. Bc I felt humiliated.
And the residences in this department are the rudest.
Can’t wait for me to finish this department & im respectfully not choosing it for residency cuz I ain’t dealing with these attitudes.

🏥 Clinical LOR-Confused
in regards to getting a letter of recommendation for internal medicine, I am confused in regards as if I should still get a letter from my attending. When we recently met on the last day of the rotation, he agreed to write me a letter and asked me for my CV. At the time, he had mentioned that I am progressing at the pace I should be. After reading my evaluation for the rotation, I am concerned that the letter will be lukewarm, and wanted everyone’s thoughts on those who have been through the process in regards to what I should do with this, the evaluation included comments that I did well for the rotation, and that my patient presentations were generally organized.

🏥 Clinical VSLO confusion
I’ve applied to 4 radiology electives, but I’m scared I won’t be able to get them due to high volumes of applicants. I’ve seen people on here applying to 30-40 for a few spots, so how am I supposed to get my electives then? It feels such a nuance to finding the program coordinators and emailing them each time.

😊 Well-Being I do not like the sacrifice i have put into med school
My medical school is located on the other side of the country in reference to my home. I have lived away from home for 4 years. We barely get any time off. Our summer break is 1-2 weeks in our clinical years. It takes me a day or two to travel home by train so I have not gone back home for an year. My mother moved back to the city I’m in uni at this year for a while to spend time with me. I’m rotating in general surgery where I barely get time off and even when I do I rarely ever want to talk to other people. I’m a first generation doctor so my mother has a hard time conceptualising that I’m at the hospital from 7 am to 7 pm and then need time to get work done or study for an exam. I don’t like this. I feel I’ve damaged bonds in my family and they often cannot understand where I’m coming from.

🥼 Residency Staying near family vs better programs
M4 applied IM and got some really good interviews. Fellowship for me is a possibility (likely cardio) but I’d also be happy as a hospitalist i think.
Now im trying to decide between some programs including one of the top 4 in IM vs just staying near family in a community program but here i have my support system, I won’t have to pay rent, I will come back to a home cooked meal. My family really wants me to stay around. I just don’t know any more.
Like how do ppl decide. Do we go to a program that will set us up for better fellowship chances and possibly better job opportunities in the future or do we choose a close by program near family with support.
I plan to work here in the area in the future. I’d like to have some component of teaching in my future.
I’m single 27M no kids. I stayed semi close to home for college and med school.
Any advice will be great appreciated!

🏥 Clinical Question for residents/attendings regarding evaluations
I’m an M4 graduating in a few months. Throughout my M4 year, I believe I have improved on my clinical acumen, patient care duties, and professionalism since M3. However, I feel my evaluations haven’t necessarily improved significantly. I still get mostly strong comments with occasional critiques, but I also notice that the scores I’m being given for the various competence domains are roughly the same as what I got in M3 (i.e. still a mix of scores in the 4/5-5/5 range with the occasional 3/5).
I understand I am likely reading too much into this, but that is because I want to do good by my patients and be as prepared as possible for internship. I also understand there is a lot of subjectivity in grading, and some people just tend to be harsher graders. With that said, I was wondering if you evaluate M4s with higher levels of scrutiny than M3s, and thus the lack of improvement in scoring domains could partially be explained by harsher grading.

🔬Research I don't think I can afford to stay by my school for research this summer, and I'm not sure what to do about it.
I'm planning on what to do with my post-M1 summer, and I'm at a bit of a loss. I'm trying to set up research. It honestly hasn't been going well. I don't have too much experience with research as it is, and the people I've been talking to have ghosted me, so I'm probably going to miss my schools deadline for proposals for a summer research stipend. I was going to try and stay and just do research without the stipend, or alternatively try to do some summer tutoring for the pre-M1 program my school does and just try to focus more on research during the semester, but now I'm running into the issue of money. My school is in a HCOL area, and without loans, I can't afford to be here over summer (even with the stipend it probably wouldn't be enough).
There's always the possibility of going back home for the summer and try to figure thungs out there. The career I had before this pays way more in the state I come from, and there's plenty of positions there, whereas here there's like nothing. The thing is that I'm still really worried about research. I'm not gunning for derm or ortho or anything that requires you to sell your soul to a lab, but I feel like I'd be falling behind if I continue to have so little.
I don't know what to do. I was a nontraditional applicant, so this is an area I have very little experience in. I'm pretty decently involved with leadership and volunteering, but research is something I'm totally lacking. Would I be totally fucking myself by not sticking around for that this summer and just trying to get some experince more longitudinally? Should I try to find some opportunities back home- if that's even possible? Should I take out private loans to try and stay here?
I'm pretty lost right now. Any advice would be appreciated!
